All employees must be protected from falling through holes by fall arrest systems, guardrails or covers. Employees are also to be protected from tripping or stepping into holes regardless of the depth.

All floor/roof holes and/or other openings must be securely barricaded or covered and clearly marked “Hole”. Never leave an open hole unprotected and DO NOT remove any hole cover or barricade without proper fall protection as outlined by a specific fall protection plan.

When creating an open hole on an elevated area, every measure must be put in place and every precaution taken to ensure the protection of people and facilities from falls and from falling tools or material to lower levels.
